1. Clinical application of closed suction catheter

Mechanical ventilation can significantly reduce the mortality rate of patients with respiratory failure, but it causes the epiglottis to temporarily lose its function, the cough reflex is weakened or suppressed, and more sputum suction complications are likely to occur, which greatly affects the effect of treatment. Therefore, safe and effective sputum suction operation is an important part of the nursing work in the intensive care unit, and an important method to ensure adequate ventilation and ventilation of patients.

Mechanical ventilation has been widely used in the rescue of critically ill patients. After the artificial airway is established, affected by factors such as disease and analgesia, sedation, etc., the patient's ability to expel sputum decreases, and a series of complications such as pulmonary infection are prone to occur.

Therefore, mechanical suction has become a necessary measure to ensure airway patency in mechanically ventilated patients.

The closed suction catheter, artificial airway and ventilator are jointly constructed to form a special closed action system, which can effectively maintain the patency of the artificial airway during the suction operation, ensure sufficient ventilation and gas exchange, and provide good mechanical ventilation.

         

          2.1 AARC 2010 (American Airway Aspiration Guidelines)

AARC 2010 stipulates that the outer diameter of the suction tube for adults should not exceed 50% of the inner diameter of the tube, and that for children and infants should not exceed 70%

          2.2 Model selection survey

  Investigate INTERSURGICAL, Chilecom, and AVANOS, and summarize the following three selection methods for closed suction tubes:

(1) INTERSURGICAL: the outer diameter of the suction tube does not exceed 1/2 of the inner diameter of the intubation tube;

(2) Chilecom: the outer diameter of the suction tube is about 1/2 of the inner diameter of the intubation tube;

(3) AVANOS: After intubation inner diameter * 2, select the outer diameter of the next-smallest type of suction tube (e.g. 8mm inner diameter of the intubation tube should choose 8*2 times the small size of the 14Fr suction tube)

Through the above three methods, the intubation models corresponding to different types of sputum suction tubes can be obtained, which are summarized in Table 1:
